Objective: To describe the management of a 35-year-old male with low back pain and radiculopathy, left lower extremity weakness, and fatigue with activity.

Clinical Features: The patient’s pain was initially mild and achy, but pain increased with physical activities such as jogging. He also experienced worsening weakness and fatigue in his left lower extremity toward the end of his runs. His medical history was unremarkable, with no significant comorbidities or previous surgeries. Neurological and orthopedic examination was normal, though he had mildly elevated blood pressure. Diagnostic imaging did not demonstrate any abnormalities. CTA with runoff diagnosed the presence of renal fibromuscular dysplasia (FMD).

Intervention and Outcome: We collaborated with a vascular surgeon in the management of this patient. Chiropractic treatment promoted functional movement and improved lower extremity circulation. This included chiropractic manipulation, soft tissue mobilization, and an active home care plan. He was provided supplementation and home dietary recommendations to support vascular health and normal blood pressure. The vascular physician determined that no medications or surgical intervention was necessary but the patient was asked to monitor his blood pressure and continue regular imaging.

Conclusion: The rarity of this condition leaves data sparse other than in national FMD registries. Enhanced awareness with case presentations will allow physicians to make early detection and diagnosis possible and prevent complications with patients and their families.
